INFORMED CONSENT FOR SPORT MEDICINE MODALITIES

I understand that sports medicine and rehabilitation therapy utilizes multiple modalities that are relatively new to the field of veterinary medicine. Examples include manual therapy, acupuncture, extracorporeal shockwave therapy, photobiomodulation (laser), and regenerative medicine (stem cells or platelet rich plasma). The College of Veterinarians of British Columbia classify many of these treatments as complimentary or alternative medicine. Points East West Veterinary Services considers them to be a form of integrative medicine (to be used alongside conventional therapies), and an important standard of care.
In instances where canine specific published research for a given modality is limited, this information will be communicated to you by one of our veterinarians. Points East West Veterinary Services adheres to the principals of evidence based medicine.
I agree to the use of these therapies with the understanding that they may aid in the treatment of my animal's condition. I realize that there can be no guarantee as to the animal's condition or the outcome of any procedures. As the owner of the animal described below, and being eighteen years of age or older, I hereby authorize Points East West Veterinary Services to treat my animal with these therapies. I have read this authorization form, and understand it and give my consent.
